This potentially fatal side effect occurs when opioids suppress the brain’s ability to regulate breathing, leading to inadequate ventilation and, in severe cases, respiratory arrest. Understanding the underlying mechanisms is essential for developing safer analgesics and effective interventions.

Opioids exert their effects primarily by binding to mu-opioid receptors (MORs) located in the central nervous system. These receptors are G-protein-coupled receptors that, when activated, inhibit neuronal excitability. In the context of respiration, MORs are densely expressed in the brainstem regions responsible for respiratory rhythm generation, including the pre-Bötzinger complex and the ventral respiratory group.

Activation of MORs in these areas leads to hyperpolarization of respiratory neurons through increased potassium conductance and decreased calcium influx. This results in reduced neuronal firing rates, diminishing the respiratory drive.
